DVA Claim Forms & Documents

DVA requires specific forms depending on your claim type. Some of the most common include: 

D2020 – Compensation Claim

Purpose: Lodge a claim for service-related conditions (physical, mental).

Eligibility: Veterans or dependants who believe they have a condition caused or aggravated by service.

Required Documents:

  • Proof of identity

  • Service records (ADF documentation)

  • Medical evidence linking the condition to service

  • Specialist/GP reports

  • Any previous DVA decision letters (if re-lodging)

D9319 – Claim for Household Services

Purpose: Arrange or reimburse domestic assistance (e.g. cleaning, gardening) due to service-related incapacity.

Eligibility: Veterans with an accepted service-related condition that prevents them from reasonably performing household tasks.

Required Documents:

  • Completed claim form

  • Medical certificate or GP statement confirming incapacity

  • Description of required services (tasks & frequency)

  • Quotes or invoices from providers (if reimbursement is sought)

  • Proof of identity and DVA file number

D1217 – Claim for Treatment Services Voucher

Purpose: Access vouchers for allied health, community nursing, or other approved treatment services.

Eligibility: Must be an eligible DVA cardholder; treatment must be DVA approved; provider must be DVA registered.

  • Completed voucher form

  • GP or specialist referral (if required)

D0800 – Travel Expenses Claim

Purpose: Reimbursement of travel costs to attend DVA-approved medical appointments.

D0501 / D0503 / D0648 – Service Pension / Income Support Pension Forms
