DID NOT ATTEND (DNA) & CANCELLATION POLICY

For the benefit of our Doctors and other patients, we appreciate you notifying us 2 hours before your appointment time if you no longer need or would like to re-schedule your appointment. If a patient fails to attend, in accordance to our “Did Not Attend” policy, DNA fee will be charged to your account.

DNA Fee: $50 for all appointments

If this amount if not paid a letter of suspension of services and the outstanding account will be issued and access to all services suspended until account is paid.

There may also be the possibility of permanent suspension from the practice at the Doctor’s discretion for repeat offenders.
