This Privacy Policy applies to all personal information collected by GreenCanna Health Pty Ltd (ACN 664 477 454) via the website located at http://hpi.71b.myftpupload.com.

    This is the official Privacy Policy of CannaAid Medical operated by GreenCanna Health Pty Ltd (“CannaAid Medical”, “GreenCanna Health”, “we”, “our”, “us”). This policy explains how we collect, store, use and disclose personal information and also explains your right to access or to make a complaint about our handling of personal information.

    GreenCanna Health Pty Ltd collects personal information from you in a number of different ways. We may collect personal information directly from you or in the course of dealings with you, for example when you:

    GreenCanna Health Pty Ltd may use or disclose your personal information for the purpose for which it was collected. We will also use and disclose your personal information for a secondary purpose that is related to a purpose for which we collected it, where you would reasonably expect us to use or disclose your personal information for that secondary purpose.

    You also consent to:
    • CannaAid Medical accessing and uploading to your My Health Records
    • Digital script sharing via email and SMS
    • CannaAid Medical using email services to communicate with you about your medical information. (GreenCanna Health Pty Ltd takes no responsibility for cases where incorrect emails have been supplied or where emails being supplied are not
    registered to the patient)

  5. Information about events, the Website and our services
    We may contact you via email, SMS or other means in order to provide you with updated information about the Website, in relation to events or to provide you with other information about our services. If you do not wish to receive any such information, please contact us as set out below.

  6. Security of Personal Information
    GreenCanna Health will take reasonable steps to keep any personal information we hold about you secure. However, except to the extent liability cannot be excluded due to the operation of the statute. GreenCanna Health excludes all liability (including in negligence) for the consequences of any unauthorised access to your personal information. Please notify us immediately if you become aware of any breach of security.

    We may store your files in hard copy or electronically in our ordinary IT systems. These may include Australian-based cloud servers or the servers of third parties within Australia. When we have disclosed your personal information to third parties and your files may be stored by or transferred to entities located outside of Australia. The storage or transfer of your personal information by these third parties will be subject to confidentiality provisions as we deem appropriate.

    We implement a range of physical and electronic safety measures to protect the personal information that we hold. Including;
    • Mandatory password protection on all computers
    • Firewall and antivirus/malware software

    Staff receive periodic training on security issues, to foster a security aware culture. We also have a regular review program to test the security measures in place and identify where changes may be necessary or desirable.

  7. Overseas transfer
    Your personal information will not be disclosed to recipients outside Australia unless you expressly request us to do so. If you request us to transfer your personal information to an overseas recipient, the overseas recipient will not be required to comply with the Australian Privacy Principles, and we will not be liable for any mishandling of your information in such circumstances.

    If you believe that we have breached the Australian Privacy Principles and wish to make a complaint, please contact us using the details below and provide us with full details of the alleged breach. We will promptly investigate your complaint and respond to you, in writing, setting out the outcome of our investigation and the steps we will take to deal with your complaint.

  12. Cancellation Policy
    For the benefit of our Doctors and other patients, we appreciate you notifying us 2 hours before your appointment time if you no longer need or would like to re-schedule your appointment. If a patient fails to attend, in accordance to our “Did Not Attend” policy, DNA fee be will charged to your account.

    DNA Fee: $50 for all appointments

    If this amount if not paid a letter of suspension of services and the outstanding account will be issued and access to all services suspended until account is paid.

    There may also be the possibility of permanent suspension from the practice at the Doctor’s discretion for repeat offenders.
